A 23-year-old man attends the urgent clinic with a very sore throat, increasing over two days. The pain is so bad he can barely swallow and has trouble speaking. Examination is difficult: he can barely open his mouth. You can just about see obvious swelling to one side. He is feverish and looks ill.
